This policy does not apply to protected health information (PHI) collected while you are a patient or seeking treatment. PHI is governed by Part II of this document — our HIPAA Notice of Privacy Practices.

Part II — HIPAA Notice of Privacy Practices

Your Information. Your Rights. Our Responsibilities. This section applies to patients receiving care through the Stormtech26 LLC Platform.

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Who Provides Your Care

Medical care through the Stormtech26 LLC Platform is provided by OpenLoop Healthcare Partners, PC and its affiliated professional corporations (including OpenLoop Healthcare Partners California, PC; and Untitled Health PLLC), which operate as a HIPAA Affiliated Covered Entity (“ACE”). These entities may share your protected health information (PHI) with each other as needed to carry out treatment, payment, and healthcare operations. A single Privacy Officer is responsible for HIPAA compliance across all ACE participants.

Stormtech26 LLC is a business associate of these covered entities and handles PHI pursuant to a Business Associate Agreement, consistent with HIPAA requirements.

Your Rights as a Patient

Access Your Records

Request a copy of your medical record or other health information we hold about you.

Correct Your Records

Ask us to correct health information you believe is incorrect or incomplete.

Restrict Use

Ask us not to use or share certain health information for treatment, payment, or operations.

Confidential Communication

Ask us to contact you in a specific way or send mail to a different address.

Disclosure Accounting

Request a list of who we’ve shared your health information with and why.

Designate a Representative

Treatment

We use your health information to provide care. We may share it with other licensed clinicians involved in your treatment.

Payment

We may use and share your health information to bill and receive payment for your care.

Healthcare Operations

We may use your information to run our practice, improve your care, send appointment reminders, and contact you when necessary. We may use AI and machine learning tools to analyze health information and improve clinical workflows, in compliance with HIPAA and applicable law. These tools do not replace licensed clinician judgment.

Other Permitted Uses

We may also share your information, to the extent permitted by law, for:

  • Public health and safety issues (e.g., disease prevention, mandatory reporting)
  • Health research (under appropriate safeguards)
  • Legal compliance
  • Organ and tissue donation requests
  • Medical examiner or funeral director purposes

Uses Requiring Your Written Authorization

  • We will notify you promptly — and no later than 60 days after discovery — if a breach occurs that may have compromised your information.
  • Where state laws require greater privacy protections than HIPAA, we follow state law.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing. You may revoke that permission in writing at any time.

Changes to This Notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request and on this website.
